GPHA provides complete diagnostic and management services for all chronic diseases.
Due to the high number of patients with diabetes, GPHA is proud to offer its Diabetes Center of Excellence which includes the services of a staff Endocrinologist who works in collaboration with the patient’s primary care provider and other members of the clinical team to lower the hemoglobin A1C to levels that are associated with reductions in complications such as heart attack, stroke, kidney failure, blindness and amputation.
GPHA has been a leader in HIV care since 1981. In recognition of the continuing need for comprehensive HIV care and the growing problem of viral hepatitis (hepatitis B and C), GPHA created the Infectious Disease Center of Excellence, dedicated to the diagnosis, control and cure of infections. Comprehensive care is provided for patients with blood borne diseases such as hepatitis C, hepatitis B and HIV infection as well as a host of other infectious diseases. GPHA’s professional staff includes two staff Infectious Disease specialists.
Both Centers of Excellence collaborate with the Nutrition, Podiatry, Dental, and Behavioral Health Services as well as patient education and support through our dedicated team of nurses, medical case managers care managers and clinical assistants.
