Hunting Park Health Center Renovation Frequently Asked Questions
When does Hunting Park Health Center (HPHC) close for construction?
The last day you can be seen at HPHC is Friday 1/27/2023. Beginning Saturday 1/28/2023, HPHC will be closed for construction.
When will the newly renovated HPHC open?
Construction is expected to take about 12 months.
Can I continue to receive care from HPHC providers? Will I see my same provider?
Yes, you can still see your usual provider. HPHC medical providers will continue seeing patients at Frankford Avenue Health Center (FAHC) and the Temple Medical Suite locations. Dental and Behavioral Healthcare services will be provided at FAHC and other locations. Please refer to the attached list of GPHA locations for the facility addresses.
Can I see a different provider or go to a different GPHA health center?
Yes. HPHC patients are welcome to receive services at any GPHA facility.
Do I need to transfer my records?
There is no need to copy or transfer medical records. All GPHA health centers are linked to the same electronic health record.
How do I schedule an appointment?
Please call our Customer Care Center at 1-888-296-GPHA (4742).
Can I still get telehealth and face-to-face visits?
Both in-person and telehealth visits are available at all GPHA locations.
What will happen to my prescriptions?
Your prescriptions will continue to be sent electronically to the pharmacy of your choice. If you have prescriptions at the HPHC pharmacy that have not been picked up by January 27, 2023, you will have the option to have them delivered to you, mailed to
you or held for you to pick up in-person at the FAHC pharmacy.
Pharmacy services are available at all GPHA locations.
I’m interested in transportation assistance. What do I do?
If you are covered by Medicare or Medicaid, your insurance company may provide free transportation. Simply call ModivCare
877-835-7412 and press “1”. Have your insurance card ready; the operator will need your insurance number.
If you would like to use public transportation, you may qualify for the Medical Assistance Transportation (MAT) Program, which provides free Septa “keys” that allow riders to use public transportation (trolley, train, and bus) at no cost for all kinds of medical, pharmacy and other health-related needs. Call 866-550-4355 to apply for this service.
If you are interested in Lyft transportation, please complete and sign the Circulation Consent Form. You may pick up a consent form at the HPHC Registration Desk. Please return your consent form to our front desk OR you may mail it to our administrative office:
