A GPHA patient may also report any patient safety concerns to The Joint Commission Quality and Patient Safety Office. To report a patient safety event or concern online, submit a new patient safety event or concern or submit an update to your incident. (You must have your incident number.) You may also submit the event or concern via mail, telephone, fax or email.

Urgent Medical Care/After-Hours Coverage
If you are a GPHA patient or family member, call the site where you received medical attention. If you need medical, dental, mental health or substance abuse help after hours, leave your name, phone number and concern with the on-call answering service. Your call will be returned by a GPHA provider.
Some of our GPHA sites are open evenings as well as weekends. Please review all GPHA health center hours of operations. You can be seen at any facility that is open. The on-call provider will arrange for you to be seen at one of the open locations.
If this is a true medical emergency, please call 911 or go to an emergency room (ER) immediately. Please remove all call-blocking codes and/or devices while awaiting a doctor’s call after hours. Please note: On-call providers do not prescribe or refill narcotics or benzodiazepines.
Atención médica de emergencia/servicios de guardia en hora no laborales
Si usted es un paciente GPHA o un familiar llame al Centro de Salud integral donde recibió atención médica. Si usted necesita ayuda médica, dental o terapia de salud mental durante horas no laborales, deje su nombre, número de teléfono y motivo de consulta con el servicio de guardia. Su llamada será devuelta por un Medico de GPHA.
Algunos de nuestros Centros de Salud Integral laboran durante la noche y los fines de semana. Por favor revise todas las horas de operaciones de los Centros de Salud Integral de GPHA. Puede ser visto en cualquiera de los Centros de Salud Integral que esté abiertos. El Medico de Guardia hará arreglos para que usted sea visto en uno de los centros abiertos.
Si esto es una emergencia médica de verdad, por favor llame al 911 o vaya a una sala de emergencias medicas (ER) inmediatamente. Por favor, elimine todos los códigos y/o dispositivos de bloqueo de llamadas mientras espera la llamada de nuestro médico de guardia.
