Dr. Samuel Kotoh is Board-certified in Internal Medicine. He received his Bachelor of Medicine Science degree in Physiology/Pharmacology from the University of Nottingham Medical School in 1986 and his Bachelor of Science degree from the University of Science and Technology in Kumasi, Ghana, in 1988. He completed his residency training at Columbia University College of Physicians and Surgeons at Harlem Hospital, NY, in 1995. He joined GPHA in September of 2002.
Dr. Monica Mallory-Whitmore is Board Eligible in Family Practice in July 2006. She received her Doctor of Osteopathy from New York College of Osteopathic Medicine in May 2003. Her Residency training in Family Practice was completed at Mercy Suburban Hospital. Dr. Mallory-Whitmore joined GPHA’s Medical Staff in 2006.
Dr. Fredric Paul Nelson
Dr. Fredric Paul Nelson is a Diplomat of the American Board of Pediatrics.He received his Medical Degree from the University of Minnesota, Minneapolis and completed his residency and internship training at BostonFloatingHospital, Tufts-NewEnglandMedicalCenter, Boston, MA.Dr. Nelson joined GPHA’s Medical Staff in 2005.
Dr. Brenda D. Rogers is Board-certified in Internal Medicine with extensive experience in general Internal Medicine and HIV clinical care. Dr. Rogers received her medical degree from the Medical College of Pennsylvania and completed her internship and residency in Internal Medicine at Hahnemann University Hospital in Philadelphia. Dr. Rogers joined GPHA's Medical Staff in 1993. She provides clinical services to the adolescent and adult population and also serves as GPHA's Associate Medical Director.
Dr. Heather Ruddock provides well-childcare as well as acute and chronic disease management for babies, children and teens. She has a special interest in early childhood development and is Board-certified in Pediatrics. A graduate of Columbia University, she received her medical degree from Temple University School of Medicine and completed residency training in Pediatrics at St. Christopher’s Hospital for Children. She joined GPHA’s medical staff in January 1998.
Dr. Chinedum U.S Udenze-Utah Dr. Chinedum N.S Udenze-Utah received her Internship training from the University of Lagos Teaching Hospital, Lagos, Nigeria and Residency in Pediatrics from the Medical College of Pennsylvania. Dr. Udenze-Utah received a Doctorate degree in Medicine and Surgery from the College of Medicine Science, University of Benin, Benin City, Nigeria. Dr. Udenze-Utah is board certified by the American Board of Pediatrics and joined the Medical Staff of GPHA on 2006.
Patricia Weir Asthma Specialist Educator, has been a registered nurse since 1969. Asthma is Ms. Weir’s specialty area, including intensive patient education and asthma treatment. She graduated with her master's degree and Family Nurse Practitioner certification from Rutger's University in 2001. She received her asthma training at Penn Lung, Children's Hospital of Philadelphia, Temple University and Temple Pediatrics. Ms. Weir joined GPHA’s medical staff in March 2002.
Carol H. Allen received her medical degree from the State University Hospital, Buffalo, New York. She completed her post graduate residency training at Hahnemann University Hospital, Philadelphia, PA. Dr. Allen has her Pennsylvania State Medical License. Dr. Allen joined GPHA’s Medical Staff in 2010 to provide OB/GYN services at GPHA Heath Centers.
Juan Diaz graduated form the Instituto Technologico de Santa Domingo. He completed his post graduate residency training at Temple University, Philadelphia, PA. Dr. Diaz has his Pennsylvania State Medical License. He joined GPHA’s Medical Staff in 2010 to provide OB/GYN services at Woodland Avenue Health Center.
Dr. Jeffrey K. Dunbar is board certified in Internal Medicine. He completed his post baccalaureate program from Bryn Mawr College, Philadelphia, PA. Dr. Dunbar received his medical degree from the Medical College of Pennsylvania, Philadelphia, PA. He also received his post graduate training from John Hopkins/Sinai Hospital Program in Internal Medicine, Baltimore, Maryland. Dr. Dunbar joined GPHA’s Medical Staff in 2009 at the Woodland Avenue Health Center.
Dr. Carrie Jordan received her medical degree from the Pennsylvania State University College of Medicine, Hershey, PA. She completed her Residency training in Internal Medicine at Montgomery Internal Medicine Residency Program. Dr. Jordan joined GPHA’s medical staff in 2007 as an Internist.
Vida Mia Ruizholds a Masters of Science, Nurse-Midwifery degree from the University of Pennsylvania , Philadelphia, PA. Ms. Ruiz received her Bachelor of Science, Nursing from George Mason University, Washington, D.C. Ms. Ruiz joined the professional staff of GPHA in 2009.
Christine Watts holds a Masters of Science in Nursing degree from Case Western Reserve University. Ms. Watts has also received her Certificate in Nurse Midwifery from the Frontier School of Midwifery and Family Nursing. Ms. Watts is a Certified Nurse Midwife. Ms. Watts joined GPHA’s Medical Staff in 2010.
Sesame Worku
received her BS in Physician Assistant from MCP Hahnemann University, Philadelphia, PA. Ms.Worku completed training as a Physician Assistant at the University of Nebraska, Omaha. Ms. Worku joined GPHA’s Medical Staff in 2008 at the Woodland Avenue Health Center. She provides comprehensive preventive care and acute and chronic disease management for patients of all ages.
